Registration Information

ELA Participant Requirements

  • Be 22 years of age and older or a high school graduate.
  • Complete the intake assessment process to determine ELA eligibility and participant program needs.
  • Submit participant information forms as requested by the ELA program.

Registration Process

  • Complete the Adult Day Program Interest Form
  • The Recreation Manager for ELA will contact the interested family to schedule an assessment.
  • Participants, legal guardians and other professionals are involved in the assessment process prior to a participant’s enrollment in the ELA Program. During the assessment, information is gathered through the Therapeutic Recreation Activity Assessment (TRAA) and a parent/professional assessment questionnaire to determine the level of support and goals for the participant as they begin the program.
  • Once the assessment is completed, the participant is placed on the interest list with their staffing support ratio and desired registration option. Due to the financial cost of the Program, there is a limit of three (3) registrations per day for registrants who assess at a 1:1 staff to participant ratio. If there is a program opening, and three 1:1 registrations are filled, a registrant may choose to provide a private companion within established limits.
  • Staffing is unique to each participant based on needs determined by the assessment.
  • The participant will be placed on the interest list if the preferred program registration is unavailable after the assessment process.
  • When a registration opening becomes available, those on the interest list will be contacted in the order they were added.

When a 1:1 registration opening becomes available, all individuals on the interest list who assessed at the 1:1 ratio will be contacted to be entered into a random drawing to acquire the open registration.

  • When a 1:1 opening becomes available, a random drawing will occur for available registration slots if the number of openings is fewer than the participants on the interest list.

ELA accepts registration throughout the program year. For openings, questions, or more information, contact Lacey Allen, Recreation Manager for ELA, at (847) 509-9400 x6833 or lallen@nssra.org.
